Your Appointment Info:

ALL APPOINTMENTS ARE HANDLED BY PHONE. DO NOT COME TO THE OFFICE.

 

Arrive at your appointment on time and with the following items:

  1. Photo ID of applicant for all adult household members
  2. Social Security Card or ITIN numbers for all household members
  3. Birth Certificates for all children age 18 or younger
  4. Current Electric Bill
  5. Current Lease/Rental Agreement listing all household members & address
  6. Home owners need to provide current Property Appraiser Record or Property Tax Record
  7. Proof of all current income/benefits for ALL household members including all of the following that apply:
    • Last 4 Weeks of Employment Pay Stubs
    • Proof of Self-Employment Income
    • Unemployment Benefits
    • 2019 Social Security / Disability / SSI Award Letter
    • Current Retirement or Pension
    • Alimony / Child Support (If court ordered, but not receiving child support, a case history is needed to show proof of non-payment)
    • All household adults without any income need to complete a Self-Declaration of Income form and provide photo ID at the time of appointment, or have the form notarized outside of StepUp Suncoast.
    • Current Food Stamp /TANF/ Medicaid Award Letter
    • HUD / Section 8 HAP Agreement & Utility Reimbursement Payment
    • HUD Calculation Summary Sheet
